Advanced Care Planning workshop will be presented by Lindsey Boldrin of PeaceHealth

Join us for an interactive in-person workshop on completing a Washington State Advance Directive. All participants will be guided through this simple yet thought provoking  process of choosing a health care agent and putting your wishes in writing. A Notary Public will be present to notarize the document for participants during the session. Presented by the PeaceHealth Advance Care Planning program.
